New Patient Information Forms rev 809
New patients are asked to arrive to our office 20 minutes prior to their appointment time. We will need your insurance cards and photo ID upon arrival. If the patient is a minor we will need the parents or legal guardians photo ID. If you are a legal guardian we will need the notorized documents stating this. Thank you.
If you do not speak or understand English, you will need to bring an interpretor to your appointment. This person must be at least 18 years of age and be known to you personally. We will need a copy of this persons photo ID as well upon arrival. Thank you.
